Case Management Services

This program aims to support eligible individuals in achieving self-sufficiency by providing assistance with education, job search activities utility bills, transportation expenses, and other approved costs. The Case Manager collaborates with the client set clear objectives and track progress throughout the process.

Eligibility

 

Eligibility is based on household income as well as a pre-screening that a Caseworker does to determine whether or not individuals are ready to make the commitment that is required of Case Management participants.

Laquana L.

Heart & Hands
Laquana Lapoole joined the Case Management Program in 2018 as a single mother working at HEB and living below the Federal Poverty Level. While enrolled at Louisiana Delta Community College, pursuing a degree in early childhood education, she received tuition assistance from us and graduated with an associate degree in 2018.

After graduation, Laquana began working at Head Start and continued her education, earning a bachelor's degree in child development from Ashford University. Her dedication and hard work led to her promotion as the Director of the Head Start location where she was employed.

Throughout her time in the Case Management Program, Laquana received tuition, energy, and food assistance. She managed her finances effectively, enabling her to purchase a home for her family in June 2021, successfully transitioning out of the program.
